   • Chair Letter
   • Completed University of Pittsburgh Residency Certification Form/Address Change
   • Certification of English language fluency for teaching
   • Curriculum Vitae
   • Email Address Form

NOTE: Appointments at the adjunct associate professor and adjunct professor levels require review by the appropriate School of Medicine appointments and promotions committee. Faculty must be appointed with the “Visiting” prefix.
