  2. Chair Letter
  3. Copy of Offer Letter
  4. Completed University of Pittsburgh Affirmative Action Summary Form
  5. Justification letter if only one person applied or interviewed for position (if applicable)
  6. Copy of the approved position
  7. Updated curriculum vitae

Any completed employee record (er) or appointment form must be submitted with all supporting documentation and one copy of all supporting documentation
