Clinical Prefix

  • Chair's letter with justification for the appointment or promotion
  • Current CV in School of Medicine format PDF
  • 3-4 Recommendation Letters (must be equivalent to  or higher than the candidate's proposed rank)
    • at least 1-2 letters should be outside the candidates department
  • Teaching documentation/evaluation
  • Publications (OPTIONAL)

Adjunct Prefix

  • Chair's letter with justification for the appointment or promotion
  • Current CV
